How to Prepare for Your Visit

Thank you for choosing a doctor who’s part of Huntington Health Physicians. Here are a few tips to help you make the most of your appointment.

Get In Touch

Call your doctor’s office to schedule an appointment. If you’ll be undergoing tests or procedures that require planning and preparation, please find out if there’s anything special you need to do before you arrive.

Come Prepared

Be sure to bring your insurance card, photo identification, social security number, and detailed information about any medications you’re taking. Parents of first time pediatric patients should bring their child’s yellow immunization record cards. It’s also helpful to bring copies of your medical records and lab results, especially if you’re seeking a second opinion or you’re visiting our office for the first time. Before your appointment, you might want to jot down a list of questions for your doctor. That way you won’t forget anything important — and you can discuss the answers in person.

Payment for Services

All co-payments, deductibles, co-insurance and payment for cash services will be collected at the time of your visit. Payment can be made with cash, checks, credit cards or Flexible Spending Account (FSA) cards. Please review your health plan benefits prior to your visit to verify your co-payment and co-insurance amount and get answers to any questions that may arise. Be sure to ask our office what the full payment is for all cash services. If you have healthcare coverage through your employer, you’ll also need to provide your employer’s name, address and phone number.

Talk To Your Children

If you’re seeing a pediatrician, you may want to prepare little patients by explaining that regular visits to the doctor are part of staying healthy. Start the conversation a day ahead of time so you can answer questions and calm apprehensions before you arrive. You might also consider bringing a pacifier, blankie or other comforting item from home.

Cancelling or Rescheduling Your Appointment

If you need to cancel or reschedule your appointment for any reason, please call your doctor’s office at least 24 hours before the appointment.

New Patients

If you’re new to our office, please arrive 15 minutes prior to your appointment time to fill out the necessary new patient forms and health insurance information.
